I hereby give permission to Kim Johnson Photography to photography myself and/or my family members. I give permission for Kim Johnson Photography to use my photos and likeness in all forms and media for advertising, portfolio, demo, trade, stock photography, editorial, altering without restrictions, and all other lawful purposes. I understand I am entitled to no compensation. I release the photographer of all forms of claims and liability related to my photo usage. I am over 18 years of age, and I fully understand the terms of this release.

I also understand that reproducing any portrait by means of photographing or scanning (or any other form of reproduction), without written consent of Kim Johnson Photography, is considered theft and is a violation of federal copyright law and may result in legal action. I understand that Kim Johnson Photography owns the copyright to all of the images created at my portrait session. I understand that Kim Johnson Photography does not provide high resolution jpgs or any other digital format files with portrait commissioning or artwork print orders. I acknowledge that Kim Johnson Photography is not obligated under any circumstances to provide digital images.
